RELEASE OF MEDICAL RECORDS POLICY

If employed by LABOR FINDERS, I authorize the release of any of my Medical Records to you. I hereby release LABOR FINDERS and the providers of said Medical Records from any claims I may have in connection with their release. (Last Revised 04/29/2013)

RELEASE OF CRIMINAL RECORDS POLICY

I the undersigned do hereby authorize LABOR FINDERS to examine any and all criminal records and arrests on file. I understand that I am waiving my rights of confidentiality concerning my criminal history and consent to the release of criminal record files to LABOR FINDER's customers or clients.
